Aged Care Community Registered Nurse - Ballina

Description

Pay: From A$53.84 per hour

Job Description:

Registered Nurse – Community Care

Make a difference where it matters most — in the homes and lives of the people you care for.

Right at Home Northern Rivers is growing, and we are looking for an experienced and compassionate Registered Nurse to join our community care team.

If you are looking for a role that offers clinical autonomy, flexibility and genuine connection with your clients, this could be the opportunity for you.

Rather than being based in a hospital or facility, you will have the opportunity to provide high-quality clinical care in clients' own homes, building meaningful relationships and supporting people to remain independent, safe and connected to their communities.

About the Role

As a Community Registered Nurse, you will provide person-centred clinical care across the Northern Rivers, working closely with clients, families, Care Partners, GPs and allied health professionals.

Your role will include:

  • Conducting comprehensive clinical assessments and reviews.
  • Developing and reviewing individualised care plans.
  • Providing wound care and medication management.
  • Monitoring changes in health and responding appropriately to clinical concerns.
  • Supporting clients living with complex and chronic health conditions.
  • Providing clinical advice and guidance to Care Partners.
  • Liaising with GPs, allied health and other healthcare professionals.
  • Maintaining accurate clinical records and documentation.
  • Completing incident reporting and clinical follow-up where required.
  • Promoting dignity, independence, safety and person-centred care.

About You

You are a confident and compassionate Registered Nurse who enjoys working independently and building genuine relationships with clients.

You will bring:

  • Current AHPRA registration.
  • Experience in aged care, community care, primary care or a similar clinical setting.
  • Strong clinical assessment and decision-making skills.
  • Experience with wound care, dementia and chronic health conditions.
  • Excellent communication and documentation skills.
  • The ability to work autonomously while being part of a collaborative team.
  • A current Australian driver's licence.
  • Current CPR certification.
  • Relevant Police Check and NDIS requirements, where applicable.
